Title
Acute Care Medical Social Worker - Discharge Planning / Case Management

Requirements
- Minimum 2 years of clinical experience
- Required: Master of Social Work (MSW)
- Required: California state license (pending licenses not accepted)
- Charting experience in Epic

Roles & Responsibilities
- Conduct initial assessments within 24 hours of patient admission.
- Develop and coordinate discharge plans and post-acute needs for patients with varied diagnoses.
- Perform biopsychosocial assessments and crisis interventions.
- Collaborate with multidisciplinary teams, including medical and insurance case managers.
- Handle prior authorizations and referrals to post-acute resources.
- Maintain a typical caseload of 20-25 patients.

Compliance / Notes
- Orientation: 40 hours
- Pre-approved time off: 1 (per contract)
- Travelers are expected to float between facilities within the state.
- Must commit to working 3 out of 5 specified holidays.
- Scrub color: Navy or Black.
