Patient Care Coordinator

Job Purpose: - The Patient Care Coordinator at Crystal Finishing Systems is responsible for ensuring high-quality patient care by coordinating and facilitating communication between patients, healthcare providers, and administrative staff. This full-time role involves managing patient schedules, handling inquiries, and ensuring a seamless patient experience from admission to discharge. Key Responsibilities:

• Coordinate patient appointments and manage scheduling to optimize clinic operations.
• Serve as the primary point of contact for patients, addressing inquiries and providing information about services and procedures.
• Collaborate with healthcare providers and administrative staff to ensure effective communication and continuity of care.
• Maintain accurate patient records and update information in the healthcare management system.
• Assist in the development and implementation of patient care plans and follow-up procedures.
• Ensure compliance with healthcare regulations and company policies.
• Participate in team meetings and contribute to the continuous improvement of patient care services.
• Provide support for billing and insurance-related inquiries, ensuring patients understand their financial responsibilities.

• Minimum of 2 years of experience in a healthcare setting, preferably in patient coordination or case management.
• Experience working with electronic health records (EHR) systems.
• Demonstrated experience in providing exceptional customer service in a healthcare environment.

• Strong organizational skills with the ability to manage multiple priorities and tasks efficiently.
• Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and other relevant software applications.
• Ability to work independently and collaboratively within a team-oriented environment.
• Strong problem-solving skills and the ability to make informed decisions quickly.
• Knowledge of healthcare regulations and compliance standards.
