Care Planning Coordinator

Care Planning Coordinator Hours: 28 hours per week over 4 days, Tuesday to Friday. We are readvertising this opportunity as we continue our search for the right person to join our friendly team. About The Role Working closely with the Centre Manager, you will play a key role in ensuring that our care and support planning processes remain robust, person-centred, and compliant with current legislation, standards, and best practice. Your Responsibilities Will Include Developing, maintaining, and reviewing care plans and risk assessments. Supporting the delivery of high-quality, person-centred care and support. Monitoring and auditing care documentation to ensure accuracy and compliance. Promoting positive outcomes, safety, dignity, and wellbeing for service users. Supporting quality assurance and continuous improvement initiatives. Liaising effectively with families, health professionals, and external agencies. Assisting with the promotion and development of the service within the local community. About You SVQ Level 4 (SCQF Level 9) in Social Services and Healthcare, Leadership and Management, or an equivalent qualification. Experience of working in a senior care or supervisory role within health or social care. Excellent knowledge of person-centred practice, care planning, and risk assessment processes. Strong organisational and communication skills. The ability to work independently, prioritise workload, and use initiative.
