Job Summary
Physical Therapist Home Health (PT HH) North Princeton, NJ. This role involves providing outpatient home health services in a coverage territory, with a focus on patient care and treatment planning. The position requires recent home health experience and proficiency with EPIC electronic medical record system and OASIS documentation.
Job Details
- Schedules: Days shift, 8 hours per day, with a minimum of 40 hours per week.
- Certifications and Education Requirements: Must have recent/relevant Home health experience, EPIC experience, and OASIS knowledge. Experience requirement: 0 years.
- Responsibilities include providing outpatient home health therapy services in compliance with clinical policies and procedures.
- Additional Information: Coverage territory is North Princeton, NJ. Start of care and revisits are required during the assignment.
- Submit candidates with valid credentials and availability to start on or before 09/30/2026.
- Float: Float rotation may be required.
