- Clinical Oversight:
- Ensure compliance with all regulatory requirements, policies, and best practices in patient care.
- Conduct regular assessments of resident needs and develop individualized care plans in collaboration with the interdisciplinary team.
- Monitor and evaluate resident progress, adjusting care plans as necessary to meet evolving needs.
- Team Leadership:
- Supervise and mentor nursing staff, promoting professional development and adherence to best practices in patient care.
- Foster a collaborative and supportive work environment, encouraging open communication and teamwork.
- Conduct regular staff meetings to address concerns, share updates, and promote a culture of continuous improvement.
- Quality Assurance:
- Implement quality improvement initiatives to enhance resident outcomes and satisfaction.
- Review incident reports and develop action plans to mitigate risks and improve safety.
- Participate in facility audits and inspections, ensuring compliance with state and federal regulations.
- Resident and Family Engagement:
- Establish strong relationships with residents and their families, addressing concerns and ensuring they are well-informed about care processes.
- Organize and facilitate family meetings and educational sessions to promote understanding of care plans and rehabilitation processes.
- Administrative Duties:
- Assist in budgeting, staffing, and resource allocation to ensure efficient unit operations.
- Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
- Collaborate with other departments to ensure a holistic approach to resident care.
- Education: Bachelor's degree in Nursing (BSN) preferred; valid nursing license in the state of practice.
- Experience: Minimum of 3 years of nursing experience in a long-term care or rehabilitation setting, with at least 1 year in a supervisory or managerial role.
- Skills: Strong leadership, communication, and organizational skills. Proficiency in electronic health records (EHR) and care management systems.
- Certifications: Certification in geriatric nursing or nursing administration is a plus.
