Wound Nurse

Dumont Center for Rehabilitation & Nursing Care Department: Nursing Job Title: Wound care Nurse Reports To: Director of Nursing Coordinates With: All Departments Educational Requirement: Registered Nurse for at least 2 years Position Summary The Wound Care Nurse is responsible for the overall follow up of all the residents in the facility with all types of wounds. Functions Checks all new admissions and readmissions for presence of wounds. Coordinates with all Unit Managers and Nursing Supervisors for all nosocomial wounds. Ensures that treatment orders are in place. Monitors the response of the treatment orders. Weekly Wound rounds with Wound Doctor. Initiates the individual Wound care plans. Initiates and updates the weekly wound tracking and distributes to the other departments. Updates and evaluates the Wound care plans quarterly and when there is a significant change. Informs the Next of Kin of the residents with wounds on admission, readmission, and in-house developed and documents in the progress notes. Ensures that the resident with wounds have specialized mattress and off-loading devices. Coordinates with Rehab Department for positioning devices. Coordinates with the Dietician for the nutritional needs. Coordinates with the Attending Doctors for blood values and changes in the treatment orders. Coordinates with the Unit Managers and Supervisors in ensuring that treatments are done, and specialized mattresses and off-loading devices are in-place. Makes a weekly audit for off-loading devices and special mattresses and reports to the Director of Nursing her/his findings. Assists the Nursing Department as needed.
