Registered Nurse Transplant Coordinator

Overview Methodist Hospital, part of SA Health, is a leading healthcare provider in San Antonio, specializing in a wide range of medical services, from emergency care to specialized surgery. We are seeking an experienced Registered Nurse Transplant Coordinator to join our Healthcare & Medical Services team. In this vital role, you will coordinate the care of pre- and post-transplant patients, collaborating closely with surgeons, physicians, advanced practice providers, social workers, pharmacists, and other members of the multidisciplinary transplant team. You will serve as a primary clinical resource and patient advocate, guiding patients and families through the entire transplant journey, from evaluation and listing through surgery, hospitalization, discharge, and long-term follow-up. Responsibilities The Registered Nurse Transplant Coordinator is responsible for comprehensive patient assessments, development and ongoing adjustment of individualized care plans, and timely communication of clinical changes to the transplant team. They provide intensive patient and family education regarding the transplant process, medications, lifestyle changes, and follow-up requirements, ensuring understanding and adherence to complex care regimens. They coordinate diagnostic testing, consultations, and procedures, manage follow-up appointments, and monitor lab results, promptly addressing abnormalities and escalating concerns as needed. This role also involves participating in selection conferences, quality improvement initiatives, data collection, and regulatory compliance activities related to transplant outcomes and program standards.
