- BLS and ACLS - REQUIRED
- Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
- Visa Support: J1 or H1B candidates are eligible for visa sponsorship. (note in "Additional Submission Details" field)- REQUIRED
- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions
- DEA Controlled Substance License (required once hired)
- BLS/ACLS Certification (standard requirement for hospital practice)
