Job Description Job Description Job Summary We are looking for a Home Care Aides to join our team! You will be directly working with patients, following a one‑on‑one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail‑oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Prior Home Care experience is a plus. Skill(s) Moderate reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; ability to perform the essential functions of a home care aide that includes but is not limited to full use of hands, ability to stand, bend, reach, push, pull and lift between 35-50 pounds without restrictions. Position will require a current 2 step TB and screen, Pennsylvania driver’s license, a good driving record, and your own registered insured vehicle if driving clients and a criminal background check. Experience 1-2 year experience preferred. General Responsibilities The Home Care Aide assists seniors and disabled individuals who are functionally, physically, socially and/or memory impaired and need assistance and oversight. The Home Care Aide role is to assist clients with those activities of daily living that they are unable to perform without help, fostering at all times the clients' independence and freedom of choice. Qualifications 1. Meets the training requirements and demonstrates competencies as required by the DOH 2. At least 18 years of age 3. Have the required skills to perform services as specified in the participant's service plan 4. Self-directing with the ability to work with little direct supervision 5. Empathy for the needs of the ill, injured, frail or impaired 6. Must pass criminal records check; have a child abuse clearance as required 7. Complete any necessary pre/in-service training related to the participant's service plan 8. Have a valid driver's license if operation of a vehicle is necessary to provide the service #J-18808-Ljbffr
