BASE PAY $36.10 New hires may earn a salary ranging from the minimum to the midpoint of the pay scale, with education and experience taken into consideration. ABOUT THIS ROLE The Case Manager (CM) supports Plymouth residents as part of the onsite Resident Well‑Being Care team. This role provides supportive services to residents, maintains high‑quality service delivery aligned with Plymouth’s mission, and promotes housing stability through harm reduction, trauma‑informed care, and Housing First principles. This position carries a designated caseload, is assigned to an individual building, but may be reassigned based on service needs. This is a unionized position, represented by Local 8 of the Office and Professional Employees International Union (OPEIU). PRIMARY RESPONSIBILITIES Develop and maintain Individual Service Plans (ISPs) with residents to address housing stability, long‑term goals, and transition needs. Maintain knowledge of resources within Plymouth and the broader community, including public benefits, to maximize resident access. Assist residents with scheduling, transportation, and accompanying them to appointments as needed. Advocate, consult, and collaborate with community providers to ensure continuity of care. Coordinate with Housing Operations to support residents’ housing stability (e.g., rental assistance, paperwork completion, living condition improvements). Respond to crises and emergencies using trauma‑informed, harm reduction practices, contacting emergency services when required. Address resident complaints and concerns, escalating to the Residential Well‑Being Manager (RWM) as appropriate. Collaborate with the internal behavioral health team to ensure cohesive service delivery. Attend supervision and team meetings to discuss resident issues, program goals, and strategies. Work cooperatively with co‑workers and other Plymouth departments, supporting community events and aligning with building teams on resident needs. Maintain a safe and sanitary environment for residents and staff. Maintain all records, progress notes, and documentation accurately and in a timely manner, following strict confidentiality and privacy standards. Adhere to program agreements, including goals related to housing stability, eviction prevention, income/skills development, and resident self‑determination. Coordinate with internal departments for information collection, meetings, and program activities. Provide input on program policies, procedures, and improvements as requested. Perform tasks directly related to supporting residents’ well‑being and housing stability that arise during the shift. Assist with building‑or program‑specific initiatives as directed by the RWM, provided they align with the CM role. Participate in team‑based projects or efforts that support resident services or building operations, as needed. Proficient in Microsoft Office (Word, Excel, Outlook) and able to work with databases and technology tools. Attention to detail in documentation and administrative work. Willingness and ability to transport residents to appointments. QUALIFICATIONS BA/BS degree in a human service field. Advanced education can be substituted for experience and/or years of experience can be substituted for the required degree. Minimum 1 year of experience as a case manager in housing, behavioral health, or a social service setting. Experience in Permanent Supportive Housing is preferred.
