Job Summary:

This Licensed Practical Nurse (LPN) position provides bedside care to residents in a long-term care or skilled nursing facility setting. The role involves implementing patient care plans, monitoring resident health, and collaborating with healthcare teams to ensure high-quality care.

Responsibilities:

• 
  Administer medications and treatments based on physicians orders.

  Monitor residents' vital signs and report changes to supervising nurses.
  
  
• 
  Document patient care activities accurately and timely.
  
  
• 
  Assist with daily living activities and mobility support.
  
  
• 
  Ensure compliance with infection control protocols and safety standards.
  
  

Qualifications:

• 
  Valid state-issued LPN license required.
  
  
• 
  Proven experience in long-term care or skilled nursing facilities preferred.
  
  
• 
  Ability to communicate effectively with residents and team members.
  
  
• 
  Must complete required COVID-19 vaccinations or provide valid exemption documentation.

  Demonstrate strong clinical assessment and documentation skills.
