Weekend Licensed Practical Nurse-Home Healthcare

Licensed Practical Nurse (LPN) – LTCTryfacta is seeking a Licensed Practical Nurse (LPN) – LTC for our client in Rutland, VT 05702. This is a 13-week contract assignment with possible extension. Position Title: Licensed Practical Nurse (LPN) – LTCLocation: Rutland, VT 05702Duration: 13-week contract with possible extensionWork Schedule: Evenings (2:00 PM – 10:30 PM) Guaranteed Hours: 40 Hours/Week Weekend Requirement: Weekends RequiredResponsibilities for this position include, but are not limited to:Administer medications and treatments as ordered by the physicianMonitor residents' conditions and promptly report any changes to the RN or physicianProvide direct nursing care, including wound care and other clinical proceduresMaintain accurate and timely patient recordsAssist residents with activities of daily living (ADLs) as neededCollaborate with RNs, CNAs, physicians, and other healthcare team members to provide quality resident careFollow infection prevention, safety protocols, and facility policiesDeliver compassionate, resident-centered care while promoting dignity, comfort, and independenceTo be considered for this position, you should have:Minimum 2 years of Long-Term Care (LTC) experience preferredSkilled Nursing Facility (SNF) experience preferredProficient in medication administration and nursing documentationStrong assessment, communication, and patient care skillsAbility to work effectively in a fast-paced healthcare environmentGraduate of an accredited Practical Nursing (LPN) programActive Vermont LPN License (or Compact License, if applicable)BLS Certification requiredTryfacta is an Equal Opportunity/Affirmative Action Employer.
