Medical Social Worker

Sentara RMH Medical Center is now hiring a Medical Social Worker . This is a position that will be responsible for the provision of medical social work services to patients receiving care in the hospital. Education Master’s Degree – Social Work (Required) Certification/Licensure Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI within 90 days of hire (Required) Certified Case Manager (CCM) - Certification - Commission For Case Manager Certification (CCMC) within 1 year of hire (Required) Experience 1 year of case management experience (Required) 1 year of acute care experience (Required) Experience with discharge planning (Preferred) Duties/Responsibilities Collaborates with patients, families, healthcare team members, and community organizations, and using strong advocacy skills identifies needs and resources in the hospital and community. Develop and evaluates effective transition plans taking into consideration the client diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care. #J-18808-Ljbffr Sentara Healthcare Inc
