Lead Care Manager (Fully Remote)

Assess member needs in physical health, mental health, SUD, oral health, palliative care, and social supports. Develop client care plans, set SMART goals, and evaluate progress using evidence-based practices like Motivational Interviewing. Conduct outreach, arrange transportation, and maintain accurate documentation in EMR and CRM systems. Qualifications: 2+ years of experience as a care manager or community health worker. Working knowledge of government and community resources. Excellent communication skills, computer proficiency, and a valid driver's license. Remote position with required availability Monday-Friday 8:30am-5:00pm Pacific Time. Occasional travel may be required; work involves sitting, computer use, and lifting up to 25 pounds.
