We are seeking a dedicated and motivated Occupational Therapist (OT) to join our in-house subacute rehabilitation team at Riverwood Health & Rehabilitation in Starke, Florida. As an Occupational Therapist, you will play a vital role in helping residents regain independence and improve quality of life in a supportive, team-oriented clinical environment.

Key Responsibilities:

• Evaluate residents and develop individualized occupational therapy treatment plans in a subacute setting
• Provide skilled occupational therapy interventions to improve ADLs, upper extremity function, cognition, and safety
• Recommend adaptive equipment and train residents and caregivers on proper use
• Collaborate with Physical Therapists, Speech Therapists, nursing, and interdisciplinary team members
• Monitor resident progress and modify treatment plans as needed
• Maintain accurate and timely documentation in compliance with regulatory and facility standards
• Educate residents and families on therapeutic activities, safety strategies, and home management techniques

Qualifications:

• Current Florida Occupational Therapist license
• Graduate of an accredited Occupational Therapy program
• SNF or geriatric experience preferred, but not required
• Strong clinical, communication, and time management skills
• Reliable availability and strong teamwork skills
• Ability to pass background check and drug screening
