Compassionate Hospice RN Case Manager

Gentiva in Colorado Springs is looking for a Hospice RN / RN Case Manager to provide personalized hospice and palliative care to patients facing life-limiting illness. In this role, you will be the primary point of contact for patients and their families, guiding them through every step of the hospice journey. The successful candidate will collaborate with a multidisciplinary team, deliver and document skilled nursing care, and support patients' emotional needs.
