About the Role:

Join MOUNTAIN RIDGE HOSPICE LLC as an RN Hospice Case Manager. This position is for the greater Denver areas. In this pivotal role, you will provide compassionate care and support to patients and their families during challenging times.

Responsibilities:

• Conduct comprehensive assessments to develop individualized care plans for hospice patients.
• Coordinate and oversee the delivery of patient care services with a multidisciplinary team.
• Provide education and support to patients and families regarding end-of-life care options.
• Monitor and evaluate patient progress, adjusting care plans as necessary.
• Ensure compliance with all regulatory and quality standards in hospice care.
• Document patient care activities accurately and timely in electronic health records.
• Participate in team meetings and contribute to continuous improvement initiatives.
• Act as a liaison between patients, families, and healthcare providers to ensure care.

Requirements:

• Current RN license in the state of Colorado.
• Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
• Strong assessment and critical thinking skills to manage complex patient needs.
• Excellent communication and interpersonal skills for effective patient and family interactions.
• Ability to work collaboratively within a multidisciplinary team.
• Compassionate demeanor and a commitment to providing quality end-of-life care.
• Knowledge of hospice regulations and standards is a plus.
