- Weekly Guaranteed Hours: 35
- Weekend Hours: 1-5
- On-call Requirements: None
- Number of Beds: 26
- Patient Ratios: Approximately ½ census case load
- Electronic Medical Records: EPIC
- Coordinate and manage patient care across the continuum of rehabilitation services.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Conduct assessments, monitor progress, and facilitate the transition of care for patients.
- Maintain accurate documentation and records in adherence to regulatory standards.
- Required Education: Registered Nurse (RN) License
- Experience: Minimum of 1 year in Case Management
- Preferred Skills: Proficient with EPIC systems
- Personality Fit: Collaborative and flexible in a team-oriented environment
