Job Details Job Title: RN: Case Manager Location: Colorado Springs, CO Setting: Home Health Pay Range: 2190.60 - 2300.13 Schedule: M-F 8a-5p Contract Length: 13 weeks Candidate Type: Traveler Requirements: Current, unrestricted state Registered Nurse license; Associate or bachelor's degree in nursing or related field; Ability to analyze complex medical information and make rational decisions; Case Management certification. Responsibilities Perform PRN/focused and routine assessments of high risk members. Collaborate with primary care providers to develop and implement individualized care plans. Address, identify, and reassess appropriate levels of care, including non-medical support to ensure treatment compliance. Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
