Position Summary Travel Physical Therapist – Home Health (PT‑HH) in Hyannis, Massachusetts. The role involves providing skilled therapy services to adult and pediatric patients in a home health setting, organizing and conducting medically prescribed treatment programs to restore function and prevent disability. The assignment is for 13 weeks. Start Date and Schedule Start Date: August 17, 2026. Availability: 8 days per week (D/E/N). Pay Rate: $2,563.00 – $2,641.00. Location Hyannis, MA (Cape Cod). The assignment includes local travel throughout the Cape Cod area. Responsibilities Provide skilled physical therapy services to adult and pediatric patients in a home health setting. Organize and conduct medically prescribed treatment programs to restore function and prevent disability. Collaborate with interdisciplinary teams to assess patient progress and modify treatment plans. Qualifications Licensed Physical Therapist in Massachusetts. Minimum 1 year of home health experience. Current BLS‑AHA certification. SSN, DOB, and at least one reference from entire work history. Bachelor’s degree or higher in Physical Therapy from an accredited program. Willingness to work occasional holidays and 2–4 weekend days within each 13‑week contract.
