Discharge & Care Coordination Case Manager

UChicago Medicine AdventHealth in Bolingbrook, IL is seeking a full-time Care Manager for the Medical-Surgical unit. Schedule: Monday-Friday, 8:00 a.m. to 4:30 p.m., with rotating weekends and holidays, on-site at 500 Remington Blvd. The role emphasizes discharge planning, care coordination, and holistic patient advocacy throughout inpatient stays. The ideal candidate holds a Master’s degree with 2+ hospital social work experience and an active LSW or similar license, plus strong communication #J-18808-Ljbffr adventhealth
