Physical Therapist (PT)

Job Summary

We are seeking a dedicated Physical Therapist (PT) for a home health role. This position involves providing therapy to patients in their homes, with a focus on enhancing mobility and functionality. You will work with a diverse patient population, addressing various health challenges.

Key Responsibilities

• Conduct treatments in patients' homes, assisted living facilities, and other community locations.
• Manage a caseload of 3 to 5 patients per day, including evaluations and start of care visits.
• Utilize HomeCare Home Base (HCHB) for documentation and scheduling.
• Work collaboratively with rehabilitation managers and team members to ensure high-quality patient care.
• Supervise up to 2 Physical Therapist Assistants (PTAs) or Certified Occupational Therapy Assistants (COTAs).
• Adapt therapy plans according to patients' living environments and needs.
• Participate in ongoing staff development activities and training programs.
• Complete all work within designated shift hours.
• Occasional weekend and holiday work may be required.

Qualifications

• Current Physical Therapist (PT) license in California.
• Must hold a Basic Life Support (BLS) certification (AHA).
• 2 years of experience in a healthcare setting required; home health experience preferred.
• Experience with OASIS documentation is required.
• Excellent written and verbal communication skills.
• Strong organizational skills and ability to manage time effectively.

Work Environment

The physical therapist will be expected to work in various environments, including homes that may have space constraints or other challenges. Flexibility and adaptability are essential in providing care in diverse settings.

Orientation and Training

There will be a thorough 3-week orientation process, including online learning and field training. After completing the orientation, you will be expected to work independently by the end of the training period.

Additional Requirements

• Must possess a valid driver's license and proof of automobile insurance.
• Scrubs are recommended for this position; business professional attire should be worn during work hours.
