Patient Navigator (H)

The University of Miami/UHealth Department of Site Disease has an exciting opportunity for a full‑time Patient Navigator in Miami. The incumbent facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow‑up of services ordered by caregivers. Core Job Functions Conduct patient evaluations to obtain information necessary for the development of an effective treatment plan. Ensure patients have access to insurance and social service programs for which they are eligible. Provide patient reminders of upcoming appointments and act as liaison between patients and providers. Identify and resolve real or perceived barriers to care. Educate and counsel patients on issues related to their health and life situations. Maintain current knowledge of community health, welfare resources, and any other programs available to patients. Adhere to University and unit‑level policies and procedures and safeguard University assets. Core Qualifications High School diploma or equivalent. Minimum 3 years of relevant work experience. Knowledge, Skills and Attitudes Ability to process and handle confidential information with discretion. Ability to communicate effectively in both oral and written form. Ability to handle difficult and stressful situations with professional composure. Ability to maintain effective interpersonal relationships. Ability to understand and follow instructions. Primary Duties and Responsibilities (For Non‑exempt Employees Include Percent of Effort) Telephone responsibilities include, but are not limited to speaking to the patient, and contacting a variety of health care facilities to obtain patient medical record information. Address barriers and gaps in care with the nurse navigator. Provide related internal, local, or national resources to the patient, family, or caregiver. Explain consent for obtaining outside records, and HIPAA (if applicable) to patient; obtain signatures (may be done via email). Submit signed consent to health care centers as appropriate to initiate retrieval of medical records, pathology slides, x‑rays and any other pertinent report. Track pending records to ensure delivery prior to clinic visit such as pathology reports etc. Communicate to the appropriate SDG Nurse Coordinator, if difficult or unable to obtain a pertinent record. Travel within the facility and campus to retrieve records/films/CDs as needed. Prepare pathology slides and submit for consultation. Return pathology slide to appropriate institution or patient. Communicate with physician as needed. Assemble, complete, and process patients’ charts as indicated. Maintain confidentiality of patient information. Maintain an accurate clinical record, recording information per hospital policy. Perform patient call back to complete Patient Satisfaction Survey. Utilize UChart and RIS/PACS/Cerner. Report unusual incidents in a timely manner to appropriate supervisory personnel. Assist with other clerical duties, not limited to faxing, scanning, copying. Perform other duties as assigned. Knowledge, Skills, and Abilities Excellent customer service skills are required. Strong written and oral communication skills. Knowledge of medical terminology is preferred. Able to handle multiple tasks simultaneously. Education Requirements (Essential Requirements) High School diploma or equivalent. Work Experience Requirements (Essential Requirements) Three (3) years relevant experience. Experience in a medical office or hospital preferred.
