McGregor PACE (Program of All-inclusive Care for the Elderly) is a community-based service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, allowing seniors to remain at home.

As the Director of Social & Support Services, you will be responsible for direct supervision of social workers, directs social work services to assigned participants, and conducts initial assessments of program applicants. Participates in compliance issues for the frail elderly population.

Responsibilities:

• Ensures departmental compliance with PACE regulations, industry standards, and McGregor PACE Centers for Senior Independence policies.
• Oversees and manages completion of initial PACE applicant assessments and re-assessments, including psychological testing and documentation.
• Oversees and manages the delivery of community-focused participant care.
• Oversees role and functions of relevant social work staff within the inter-disciplinary team and enforces responsibilities for initial assessments, routine/episodic reassessments, plan of care development, communication with team members and documentation of planning and interventions within the medical record.
• Determines distribution of social work caseload, i.e., new participants and ongoing social work services among social work staff.
• Inter-disciplinary team support includes completion of initial comprehensive assessments of participants by enrollment date, formal reassessments with reevaluation of the care plans that are to be completed on discharges from any acute skilled admission, every six months, annually and as needed; assists in the coordination and documentation of a 24-hour care delivery for participants with input from all disciplines of the multidisciplinary team; initial and ongoing assessments of participants with a focus on physical, mental or emotional changes that are to be reported to the multidisciplinary team and reflected on the plan of care as needed.
• Participates in daily interdisciplinary team meetings and staff meetings as required.
• Participates in orientation, evaluations, and staff in-services. Presents community education programs as required.
• Communicates with participants and families regarding concerns and issues; communicates with community agencies that offer services to participants or families.
• Assists staff in setting priorities when necessary and defining goals for personal learning and staff development.
• Collaborates with managers to develop and monitor departmental budgets and expenses
• A member of the administrative team, participating in and contributing to the management team, Quality Council, and forums which develop organizational strategy and planning.

Minimum Qualifications:

• Master's degree in social work required; must be currently licensed in Ohio
• Five years of experience working w/ frail elderly in nursing facility or adult day care required;
• Minimum 5 years progressive management experience; I year exp. supervising clinical social workers
• Strong assessment skills; ability to recognize mental and behavioral problems and create and implement effective treatment plans. Ability to work with a culturally diverse population.
• Strong organizational and management skills necessary
• Strong presentation skills required

Preferred Qualifications:

• Dementia Experience preferred.
• Knowledge of PACE regulations and experience in waiver programs is highly desirable
• Basic proficiency in Windows and Word required; database and presentation applications desirable
