Position Summary
We are currently seeking a dedicated and passionate Case Manager with experience in senior living to join our team.
Case Manager Responsibilities
Orient and assist each resident as they adjust to life in the community.
Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to the community, including addressing their social, emotional, and psychological needs.
Assist each resident in maintaining family and community connections while encouraging the development of new relationships.
Complete quality management reviews to ensure the department meets state regulations and Peregrine Senior Living policies, and follow up as needed.
Collaborate with the care department and other team members to promote and protect resident rights and support each resident's psychological well-being.
Education & Experience
Bachelor's degree with major coursework in Human Services, and at least one year of full-time work experience in a related field.
Case management experience required.
Memory Care or Assisted Living experience required.
Compassion for and a genuine desire to work with older adults.
