Occupational Therapist, Home Health

Assess and screen patients' daily living and work-related skills Develop therapeutic retraining programs with measurable objectives Administer and interpret diagnostic and prognostic functional tests in patients' homes or other settings Confer with physicians and healthcare team members Participate in development and revision of Plans of Care Treatment Provide therapeutic treatment and instruction according to physician orders Improve or restore patients' strength, coordination, range of motion, and function Review and expand retraining programs according to patient behavioral goals and rehabilitation guidelines Consult vocational team members and develop vocationally oriented plans Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and families Participate in care coordination and discharge planning Recommend or design adaptive equipment and environmental improvements Supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides Document patient care observations, interventions, evaluations, interim physician orders, treatment plans, and discharge summaries Requirements Current and unrestricted OT licensure Current CPR certification Valid driver's license Auto insurance Reliable transportation Minimum six months of occupational therapy experience (preferred) Home Health experience (preferred) Patient-facing role requires TB screening Must maintain personal vehicle liability insurance in accordance with residing state minimum limits or specified higher limits Core Competencies Demonstrates expertise in developing and implementing therapeutic retraining programs, conducting functional assessments, and collaborating with healthcare teams to enhance patient outcomes.
