Role Description The Community Support Specialist is a full-time, on-site role based in Lebanon, NH. This position provides direct support to individuals living with mental and behavioral health challenges, helping them access community resources, maintain stability, and build skills for independent living. Daily responsibilities include coordinating services, supporting treatment and recovery plans, facilitating connections to social, educational, and vocational opportunities, and advocating for client needs in collaboration with interdisciplinary teams. The role involves regular home and community visits, documentation of services and progress, and ongoing communication with clients, families, and partner agencies. The Community Support Specialist contributes to a safe, respectful, client-centered environment that aligns with WCBH’s mission, values, and evidence-based practices. Qualifications Demonstrated community support experience, including coordinating services and assisting individuals in accessing resources and maintaining independence. Strong communication skills, including clear verbal and written communication, active listening, and the ability to build trusting relationships with clients, families, and colleagues. Background or education in Social Work, or Psychology, or related field with knowledge of community-based services, case management, and client advocacy. Experience working with individuals with developmental disabilities, including supporting life skills, inclusion, and person-centered planning. Additional beneficial qualifications: bachelor’s degree in social work, psychology, human services, or related field; familiarity with trauma-informed care; valid driver’s license and reliable transportation; ability to work collaboratively in multidisciplinary teams; basic computer and documentation skills. #J-18808-Ljbffr West Central Behavioral Health
