Flagstar Nursing is seeking Physical Therapists (PT) for Skilled Nursing Facilities! We are staffing Physical Therapists for Rehab Departments in state‑of‑the‑art SNFs throughout NYC and Long Island. Flagstar Rehab PTs work with a diverse and multi‑disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives. Flagstar Rehab offers the opportunity for Direct Employment with our agency or to be your own boss as an Independent Contractor. Hourly Pay Rate: $46 (W‑2), $57 (1099). Job Duties Provide efficient evaluations and create individualized treatment plans for PTAs to follow. Supervise PTAs daily. Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
