Responsibilities The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The setting may be a patient’s private residence or a patient’s room in an independent living facility. Powerback Rehabilitation To You allows the therapist to evaluate and treat in the most functional environment, which is the patient’s home. Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation. Identify the need for and make referrals to other disciplines to address patients’ comprehensive needs. Implement individualized therapy programs to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers in skills and functions essential for promoting independence and productivity. Direct activities to promote and maintain health. Provide a flexible schedule and work environment in various settings. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards. Adhere to department and patient treatment schedules, timely clinical and billing documentation, and work-related meetings. Attend and contribute to patient care, staffing conferences, and other related meetings. Consult with other rehabilitation team members to select the most appropriate therapeutic regimen consistent with each patient’s needs and capabilities. Adapt program and/or plan of care according to the patient’s needs. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Clean, maintain, identify, and report equipment repairs. Ensure patients and families receive the highest quality service in a caring and compassionate atmosphere. Screen, examine, and evaluate patients, including history, systems review, and appropriate tests and measures. Synthesize information and interpret examination findings to establish diagnosis, identify impairments, determine predicted improvement and time required to achieve it, identify precautions/contraindications, and plan care. Develop appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the treatment plan, and complete all related documentation. Apply therapeutic techniques in areas such as therapeutic exercise, aerobic capacity conditioning, balance coordination, agility training, body mechanics, postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training, functional training, manual therapy, airway clearance techniques, integumentary repair and protection, use of electrotherapeutic, physical agent and mechanical modalities, health and wellness education on chronic disease management, falls assessment and interventions to reduce fall risk, and case management in the home and community. Design, fabricate, fit and/or apply adaptive devices, and train patients in the use of adaptive devices including orthotics and prosthetics. Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level and interests. Instruct patients in necessary skills and techniques for mobility activities and evaluate progress, attitudes, and behavior related to rehabilitative potential. Perform routine re‑examination as needed to modify or progress the treatment plan. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and provide recommendations for safe mobility in the home and community space. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy, or a Master’s degree in a Physical Therapy related field plus a Bachelor’s degree in Physical Therapy, or a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience. If the qualifying Physical Therapy degree was earned outside of the U.S., it must be evaluated as equivalent to a U.S. degree in Physical Therapy.
