The Position:  

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:  

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.
• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation to ensure continuous nursing support services to our customers.
• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

Schedule & Availability Requirements

• Per Diem Position: Expected availability of approximately 10-20 hours per week , based on patient census and business needs.
• Must be available to work a flexible schedule that may include days, evenings, and occasional weekends as needed to meet patient care requirements.
• Must have availability to provide patient care on a minimum of 2-3 days per week.
• Must be knowledgeable, proficient and have used the following IV access devices to include: IV lines, accessing ports, and administration of Infusion medications (to include PIV skills, central lines, PICCS, troubleshooting infusion pumps) 
• Must be comfortable providing infusion therapy and patient education in home-based settings.
• Must be willing and able to travel within the service area, including: 
  • Routinely driving up to one hour to patient locations
  • Flexibility to travel up to two hours when needed to support patient care needs
  • Mileage reimbursement and paid drive time are provided in accordance with company policy
• Must maintain an active Nebraska RN license in good standing .

• Current Registered Nurse (RN) license in good standing with the State of Nebraska Board of Nursing .
• BSN preferred from an accredited nursing program.
• Minimum of 3 years of clinical nursing experience , with infusion therapy experience strongly preferred.
• Demonstrated expertise in peripheral IV (PIV) insertion , including the ability to successfully establish vascular access on the first attempt in the vast majority of patients (90%+ success rate preferred).
• Must be highly skilled and comfortable with accessing and de-accessing implanted ports (PACs/Port-a-Caths) and troubleshooting common vascular access issues.
• Experience administering a wide range of infusion therapies, including biologics, immunoglobulins, specialty medications, and IV hydration preferred.
• Advanced certifications in specialty practice areas (e.g., CRNI, IgCN) preferred but not required.
• ICU, Critical Care, Emergency Department, Oncology, Vascular Access, and/or Cardiac Care experience preferred.
• Strong clinical assessment, critical thinking, and patient education skills.
• Administrative Oversight: Ability to provide administrative oversight, monitor workflows, ensure compliance with company policies and regulatory standards, and manage resources effectively to meet organizational objectives.
• Emergency Availability: Must be available to respond promptly to urgent patient care and operational needs, including      occasional evenings, weekends, and holidays as required.
• Proficient with Microsoft Outlook and the Microsoft Office Suite . Experience with AlayaCare, CPR+, Insight, Concur, and Paylocity is preferred.
• Excellent communication, organizational, and customer service skills with the ability to work independently and collaboratively.
• Valid state-issued driver's license with an acceptable driving record and reliable transportation.
