Home Health LPN

Provides direct patient care, utilizing a variety of skills and coordinating services to support recovery at home. Responsible for completing documentation per policies and regulations, following the patient’s plan of care developed by a Registered Nurse, and transcribing physician’s orders. Collaborates with the care team, including nurses, patient care coordinators, and community agencies, to optimize patient outcomes. Attends patient care conferences and forwards clinical notes for coordination. Qualifications And Skills Successful completion of nursing education from an approved school. Current LPN licensure in the state of service and at least one year of clinical experience, preferably in medical/surgical settings. Ability to independently organize work and deliver effective healthcare to homebound patients. Strong communication skills (oral and written) and thorough, accurate documentation. CPR certification. Focuses on delivering compassionate, patient-centered care in a home setting, ensuring effective coordination with healthcare team members for optimal recovery outcomes. #J-18808-Ljbffr
