Days: 9:30 AM - 6 PM (Schedule can be flexible based on patient's availability). One weekend required. Job Description The Hospice Nurse is responsible for admitting patients to service and assessing the patient’s and family’s physical, psychosocial, environmental, safety, and developmental needs. This role involves developing an individualized plan of care based on assessments and documenting observations and interventions related to the patient’s environment. The Hospice Nurse coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program, and conducts follow-up visits as needed. Responsibilities Admit patients to hospice services. Perform comprehensive assessments of patients and families. Develop and document individualized plans of care. Coordinate with multidisciplinary teams to ensure quality care. Participate in quality assessment and performance improvement initiatives. Conduct follow-up visits as necessary.
