Registered Nurse (RN) - Case Management

Hospice Nurse

Schedule: Days: 9:30am-6:00pm (Schedule can be flexible based on patient availability.) Weekends: One weekend

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• One year of hospice experience

Roles & Responsibilities:

• Admit patients to the hospice service and assess the physical, psychosocial, environmental, safety, and developmental needs of patients and their families.
• Develop individualized plans of care based on assessments and document observations and interventions.
• Coordinate care with a multidisciplinary team.
• Participate in quality assessment performance improvement programs.
• Conduct follow-up visits as necessary.
