Overview Registered Nurse, Case Management – Hospice Location: Sunnyvale, California Start date: 09/08/2026 | Duration: 13 weeks | Guaranteed hours: 40/week | Shift: Days, 9:30am–6:00pm; 5x8 schedule with flexibility based on patient availability; one weekend per assignment Setting: Home healthcare – Hospice Weekly pay: $2,143–$2,278 Key Highlights Weekly pay: $2,143–$2,278 per week Guaranteed hours: 40 per week Contract duration: 13 weeks Shift: Days, 9:30am–6:00pm; 5x8 with one weekend per assignment Start date: 09/08/2026 Setting: Home healthcare – Hospice Responsibilities Admit patients to hospice services and conduct initial assessments of physical, psychosocial, environmental, safety, and developmental needs of the patient and family Develop an individualized plan of care based on the assessment; document observations and interventions related to the patient’s environment Coordinate care with a multidisciplinary team; participate in the hospice program’s quality assessment and performance improvement program Conduct follow-up visits as needed; monitor patient status and adjust the care plan accordingly Maintain clear, timely documentation in the patient record and communicate with patients and families about goals, expectations, and safety Requirements Minimum of 1 year hospice nursing experience Ability to perform in-home hospice visits and work with patients and families in home settings Strong assessment, care planning, and documentation skills Effective collaboration with multidisciplinary team and clear communication Willingness to travel within the assigned territory and maintain flexibility to meet patient needs Ability to work a 40-hour week with occasional weekend coverage Benefits and Support Dedicated assignment support from Healthforce Healthcare LLC Credentialing and onboarding assistance 24/7 recruiter and travel support Flexible scheduling considerations to align with patient needs PAY DISCLAIMER RULE Please note: Pay rates and guaranteed hours are estimates and may vary.
