Hospice Nurse

Schedule
Days: 9:30am-6:00pm (Schedule can be flexible based on patient availability.)
Weekends: One weekend

Assignment Details
Contract Length: 13 weeks
Guaranteed Hours: 40

Requirements
- One year of hospice experience.

Roles & Responsibilities
- Admit patients to the hospice service and assess the physical, psychosocial, environmental, safety, and developmental needs of patients and their families.
- Develop individualized plans of care based on assessments and document observations and interventions.
- Coordinate care with a multidisciplinary team.
- Participate in quality assessment performance improvement programs.
- Conduct follow-up visits as necessary.
