Registered Nurse, Case Management – Hospice
Location: Sunnyvale, California
Start date: 09/08/2026 | Duration: 13 weeks | Guaranteed hours: 40/week | Shift: Days, 9:30am–6:00pm; 5x8 schedule with flexibility based on patient availability; one weekend per assignment
Setting: Home healthcare – Hospice
Weekly pay: $2,143–$2,278

Responsibilities
- Admit patients to hospice services and conduct initial assessments of physical, psychosocial, environmental, safety, and developmental needs of the patient and family
- Develop an individualized plan of care based on the assessment; document observations and interventions related to the patient’s environment
- Coordinate care with a multidisciplinary team; participate in the hospice program’s quality assessment and performance improvement program
- Conduct follow-up visits as needed; monitor patient status and adjust the care plan accordingly
- Maintain clear, timely documentation in the patient record and communicate with patients and families about goals, expectations, and safety

Requirements
- Minimum of 1 year hospice nursing experience
- Ability to perform in-home hospice visits and work with patients and families in home settings
- Strong assessment, care planning, and documentation skills
- Effective collaboration with multidisciplinary team and clear communication
- Willingness to travel within the assigned territory and maintain flexibility to meet patient needs
- Ability to work a 40-hour week with occasional weekend coverage
