Responsibilities
- Admit patients to hospice service and conduct initial assessments
- Assess the patient and family’s physical, psychosocial, environmental, safety, and developmental needs
- Develop an individualized plan of care based on assessment and document observations and interventions
- Coordinate care with a multidisciplinary team and participate in quality assessment and performance improvement processes
- Conduct follow-up visits as clinically indicated and required

Requirements
- One year of hospice experience
- Ability to work in a home-based hospice setting
- Strong assessment, documentation, and communication skills
- Comfortable with travel within the designated service area as needed
