Pay Rate: $1,500 - $1,920/Week

Position Overview

We are seeking an experienced Social Worker to provide psychosocial support, care coordination, discharge planning, and resources to patients and families in an acute hospital setting.

Required Qualifications

• Minimum 2 years of Social Worker experience in an acute hospital setting.

• Active Georgia LCSW license.

• AHA BLS certification.

• Strong knowledge of psychosocial assessment, discharge planning, and community resources.

• Excellent communication, organization, and interpersonal skills.

• Ability to work effectively in a fast-paced hospital environment.

Responsibilities

• Assess patients' psychosocial needs and develop appropriate care plans.

• Provide counseling, emotional support, and crisis intervention.

• Coordinate discharge planning and post-acute services.

• Connect patients and families with community resources and support programs.

• Collaborate with physicians, nurses, case managers, and multidisciplinary teams.

• Assist with insurance, placement, transportation, and social service needs.

• Maintain accurate and timely patient documentation.

• Advocate for patients and support continuity of care.
