Youth & Family Case Manager - Behavioral Health

Apalachee Center, Inc. is seeking a CAT Care Manager to support youth and families across Madison, Jefferson, and Taylor Counties. The role emphasizes comprehensive assessments, home visits, and coordinated service planning to meet individual needs. You will develop and update individualized service plans, collaborate with families and natural supports, and document progress in Avatar.
