The Housing Specialist/Case Manager plays a pivotal role in helping LGBTQ+ people living with HIV (PLWH) secure safe, stable, and permanent housing — a foundation for health, healing, and long‐term stability.  This is not a desk‐only job. It’s hands‐on, relationship‐driven work that requires empathy, persistence, and a belief in people’s ability to rebuild their lives. You will guide clients through complex housing, healthcare, and social service systems; advocate fiercely on their behalf; and ensure they receive the support needed to remain housed and connected to care.  If you are passionate about housing justice, harm reduction, and empowering people to reclaim their lives, this role offers the chance to make a profound impact every single day.

Key Responsibilities

·         Conduct comprehensive assessments to understand each client’s needs, strengths, and housing goals

·         Develop individualized care and housing plans in partnership with clients

·         Connect clients to housing programs, benefits, medical care, mental health services, and community resources

·         Provide ongoing care coordination, warm handoffs, and follow‐through to ensure continuity of support

·         Advocate for clients within housing, healthcare, and social service systems

·         Maintain accurate documentation, prepare timely reports, and contribute to program evaluation

·         Participate in case conferences and collaborate with multidisciplinary teams

·         Support program and agency goals through additional duties as needed
