Job Details Specialty: Acute Care Case Management Discipline: RN Start Date: 06/16/2026 Duration: 13 weeks Hours per week: 40 Shift: 8 hours, days Employment Type: Travel Responsibilities Coordinate continuity of care for patients, acting as liaison between patient’s family and healthcare organization. Promote self‑managed care and cost‑effective use of healthcare resources. Ensure proper treatment is administered at the appropriate time to maximize health and well‑being. Work in settings including HMOs, community health organizations, long‑term care facilities, behavioral health programs, rehabilitation centers, schools, and case management companies. Qualifications At least 18 months recent experience in an Acute Care Case Manager setting. Active RN license. BLS certification. Degree from an accredited nursing program.
