Job Summary

Home Health PT position serving West Portland OR and surrounding areas. This role involves patient-centered physical therapy in a home health setting with efficient routing to minimize travel time. Travelers will maintain a 1:1 ratio with potential to visit up to 6 patients per day, working 30 productivity points standard.

Key Responsibilities

• Provide physical therapy services to home health patients in West Portland OR and surrounding areas
• Manage patient caseload with visits up to 6 patients per day while maintaining productivity standards
• Utilize EPIC Remote Client for Home Health (on laptop) and Rover (phone app) for charting
• Complete OASIS documentation as required by facility
• Optimize travel routes to minimize drive and windshield time
• Maintain business casual attire (scrubs allowed but not required)
• Arrange own transportation for patient visits
• Manage occasional visits with maximum radius of roughly 60 miles (60-90 minute total drive time) per day

Education Requirements

• Oregon Physical Therapist License required
• Basic Cardiac Life Support (BCLS) certification required
• OASIS experience required

Required Experience

• 1 year of skilled home health experience required
• First-time home health therapists accepted

Required Skills

• Proficiency in EPIC Remote Client for Home Health charting system
• Ability to use Rover phone application for documentation
• Experience with OASIS documentation
• Strong organizational skills for managing multiple patient visits
• Ability to optimize travel routes and manage time efficiently
• Physical capability to provide in-home patient care
• Own reliable transportation

Work Environment / Physical Requirements

• Facility requires primary series COVID vaccination in addition to one booster
• Traveler must have own transportation
• System groups clients to minimize drive and windshield time; occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day
• Business casual attire required; no jeans; scrubs allowed but not required (no specific color)
• Free parking available
• IRS Standard mileage reimbursement provided
• No weekend requirements
• No on-call requirements
• No floating between units required
• Block schedule accommodation not available

Important Notes

• Facility can call off up to one shift per pay period (every two weeks)
• Locals considered "local" within 50-75 mile radius and will be accepted at the same rate

• System does a good job grouping clients to minimize drive and windshield time
