Work Environment

Your role as a Licensed Master Social Worker will primarily be conducted remotely, allowing for flexibility in your work schedule. The position exists within a community health clinic dedicated to addressing the psychosocial needs of seniors. You will have the opportunity to collaborate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to provide comprehensive care. You may also occasionally work non-traditional hours to accommodate client needs, ensuring continuous and responsive care.

Key Responsibilities

• Conduct comprehensive psychosocial assessments of seniors to identify their emotional, mental, and social needs.
• Work collaboratively with seniors and their families to develop individualized care plans that align with their unique goals and preferences.
• Offer counseling services and emotional support to help clients navigate life transitions and cope with challenges.
• Advocate for clients’ rights and direct them to appropriate community resources, ensuring they receive quality support and service.
• Engage in regular follow-up visits to monitor client well-being, assess living conditions, and address any potential safety concerns.
• Facilitate effective communication among family members to resolve conflicts and enhance understanding regarding the care of the senior client.
• Educate clients and families on available community resources, support groups, and programs to foster social interactions and enhance life quality.
• Maintain meticulous and current documentation of all client assessments, care plans, progress notes, and interactions per regulatory and ethical standards.
• Stay up-to-date with current trends and practices in social work to ensure compliance and enhance service delivery.

Patient Population / Client Focus

You will primarily serve seniors in the Jacksonville area, addressing the unique challenges they face, including health concerns, social isolation, and emotional distress. This role emphasizes empathy and understanding while providing support and guidance to both seniors and their families throughout their journey.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW) is essential.
• A minimum of two years of experience in a social work role, ideally in settings focused on geriatrics or community service.
• Proficient understanding of aging-related issues and the array of community resources available to support seniors.
• Strong assessment, counseling, and problem-solving skills, coupled with the ability to establish rapport and trust with clients.
• Exceptional communication and interpersonal skills to facilitate teamwork and family collaboration.
• Familiarity with electronic health record (EHR) systems and technological aptitude for remote service delivery.

Preferred Experience

• Experience working in community health settings or nonprofits specializing in elder care.
• Knowledge of local resources available to seniors, including understanding Medicare and Medicaid services.
• Experience in developing and facilitating workshops or support groups for seniors and their families.

Compensation and Benefits

The salary range for this position is between $60,000 and $100,000 annually, commensurate with experience and qualifications. In addition, we offer a comprehensive benefits package that includes:
