Hospice Registered Nurse Case Manager

Suite Living Hospice Care is seeking an experienced RN Case Manager to join our hospice team. You will manage a caseload of 10–15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life. This position offers a consistent Monday–Friday schedule with a localized northwest metro territory. The role requires an active Minnesota RN license and CPR certification, with 1+ year acute care experience; hospice/home care experience is preferred. #J-18808-Ljbffr
