Orthopedic At-Home Physical Therapist – Per Diem (South Brooklyn) JAG Physical Therapy is seeking to hire a skilled Physical Therapist specializing in orthopedic rehabilitation in the home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic related conditions. Our physical therapists will be required to evaluate and treat the patients for the duration of their plan of care in their home setting with the goal of transitioning into one of our JAG outpatient orthopedic locations for continuity of care. What You’ll Need Bachelor’s or Master’s Degree required, Doctorate degree preferred Licensed Physical Therapist in the state practicing in Completed state regulated CEU requirement in appropriate state (NY, NJ) Valid CPR License Experience/knowledge with an EMR software is preferred Excellent oral, written communication Strong clinical decision‑making skills Excellent work ethic and dedication to patient success Basic computer skills Works as a team member and individually with minimal supervision Maintains a positive, professional demeanor at all times What You’ll Do Educate patient on safe bed mobility, transfers and ambulation in consideration of post operative precautions Transfer, ambulate and perform stair negotiation with patients to determine they are safe for discharge to home Collect and record patient care data that follows format and guidelines of the patient care service, regulatory agencies and third‑party payers which reflects an understanding of reimbursement systems and their impact on patient care Include but not limited to, initial evaluation, daily notes, reassessments/progress reports, discharge plans and communication with members of the team Additionally, completes daily billing and follows department guidelines for productivity Prepare for the evaluation in accordance with department policies/procedures and standards of practice including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses Select and administer tests and measures Synthesize evaluation results to formulate the diagnosis, goals, prognosis and plan of care Establish treatment plan based on evaluation results, goals and best practice. Collaborate with interdisciplinary team, patients and patient’s support system. Assess and re‑assess need for modifications to plan of care and goals. Provide education/training to patient and/or patient support system Functional Training – This includes but is not limited to ADLs/IADLs, therapeutic activities (e.g., transfers, bed mobility), rehabilitation equipment prescription and training and community reintegration Responsible for treatment, plan of care, exercise prescription for patient caseload Provide interventions to patients that are safe, effective, and in compliance with JAG Provide data, note writing using Prompt EMR for all treatments provided Attend annual JAG Billing and Coding seminar Communicate with Manager/Patient Access team for scheduling evaluations once demographics are received Important Disclaimer Notice The above statements are only intended to represent the essential job functions and general nature of the work being performed and are not exhaustive of the tasks that an Employee may be required to perform.
