Per Diem Orthopedic Home Physical Therapist - Morris County JAG Physical Therapy is seeking a skilled Physical Therapist specializing in orthopedic rehabilitation in the home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic related conditions. What You’ll Need Bachelor’s or Master’s Degree required; Doctorate preferred Licensed Physical Therapist in the state of practice (NY, NJ) Completed state‑regulated CEU requirement (NY, NJ) Valid CPR License Experience/knowledge with EMR software preferred Excellent oral and written communication Strong clinical decision‑making skills Excellent work ethic and dedication to patient success Basic computer skills Ability to work as a team member and independently with minimal supervision Maintains a positive, professional demeanor at all times What You’ll Do Educate patients on safe bed mobility, transfers, and ambulation considering post‑operative precautions. Transfer, ambulate, and perform stair negotiation to determine patient safety for discharge to home. Collect and record patient care data that follows the format and guidelines of the patient care service, regulatory agencies, and third‑party payers, reflecting an understanding of reimbursement systems. Complete initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with team members. Handle daily billing and follow department guidelines for productivity. Prepare for evaluations in accordance with department policies, procedures, and standards of practice, including obtaining patient history, exhibiting knowledge of diagnoses, and researching unfamiliar conditions. Select and administer tests and measures. Synthesize evaluation results to formulate diagnosis, goals, prognosis, and plan of care. Establish treatment plan based on evaluation results, goals, and best practice. Collaborate with interdisciplinary team, patients, and patient support systems. Assess and re‑assess need for modifications to plan of care and goals. Provide education and training to patients and/or their support system. Conduct functional training including ADLs/IADLs, therapeutic activities (e.g., transfers, bed mobility), rehabilitation equipment prescription and training, and community reintegration. Responsible for treatment, plan of care, exercise prescription for patient caseload. Provide interventions that are safe, effective, and in compliance with JAG. Document and note writing using Prompt EMR for all treatments provided. Attend annual JAG Billing and Coding seminar. Communicate with Manager/Patient Access team for scheduling evaluations once demographics are received. Required Experience Strong orthopedic background with specialization in Total Hip and Total Knee replacement rehabilitation. Education Master’s degree or higher required.
