This role is ideal for someone who is passionate about helping individuals overcome barriers, access resources, and build stable, meaningful lives in the community.As an ACT Team Case Manager, you won't be sitting behind a desk all day. You'll work directly with clients in their homes, shelters, community settings, and wherever support is needed.
