Job Requirements

• Active Alabama Registered Nurse (RN) license
• Current CPR certification required
• Associate degree in Nursing or higher
• At least 1 year of nursing experience
• Prior hospice, home health, or homecare nursing experience preferred
• Experience with HCHB EMR system preferred
• Strong clinical judgment and the ability to triage after-hours calls
• Excellent communication, organization, and time management skills

Responsibilities

• Respond to after-hours calls and provide timely clinical support
• Complete scheduled and PRN patient visits in the home health and hospice setting
• Perform admissions as needed
• Deliver patient-centered nursing care in the homecare environment
• Coordinate care with the interdisciplinary team to support quality outcomes
• Document assessments, interventions, and care plans accurately in the EMR
• Support care coverage across the assigned service area, including alternating weekends
• Provide compassionate, reliable nursing care that helps patients and families feel supported
