WORK FROM HOMETop Skills Should You Possess:Minimum one (1) year of experience in Triage, Intake or Care CoordinationWhat You'll Be Doing:Ensure complex populations get connected to the most appropriate services availableProvide the primary intake and triage services for telephonic referrals and inquiries to all care coordination programs within the organization and the external provider networkManage incoming calls to the Care Management Intake LineReceive and process inquiries for all Care Management programs from internal referrals, community providers, emergency departments, primary care providers and other agenciesProvide outstanding customer services to internal and external customersTriage and process referrals into the appropriate CM programs, actively seek consultation from clinical supervisors when needed to escalate a referral and support the Notice of Action (NOA) processEffectively manage the Community Supports services referral process including referrals from SF Medical RespiteTriage and de-escalate calls from members who may be in crisisSchedule: Monday – Friday, 8am – 5pm or 8:30am – 5:30pm (Pacific)What You Need to Bring to the Table:Associates or Bachelors Degree in related field; or 4 years of relevant experienceMinimum one (1) year of experience in Triage, Intake or Care CoordinationBi-cultural, bi-lingual language skills preferredExperience working with people with complex health needsWorking knowledge and use of Trauma Informed language and principles a plusWorking knowledge and use of Harm Reduction language and principles a plusWorking knowledge and use of Person-Centered language and principles a plusExperience with de-escalation techniquesTelephonic Case Management experience a plusExcellent verbal and written communication skillsExcellent computer skillsWhat's InIt For You?We welcome you to be a part of the largest global staffing companies to meet your career aspirations.
