Family Nurse Practitioner

Oak Street Health seeks a Nurse Practitioner to provide effective, value-based primary care to adults on Medicare in outpatient settings. You will work on a collaborative care team with physicians, PAs, MAs, a scribe, RNs, and care managers to keep patients healthy and out of the hospital.
